Superbills: A Guide to Out-of-Network Therapy Reimbursement

Rooted and Rising provides superbills on a monthly basis, by request only, for families paying privately for out-of-network speech, feeding, and occupational therapy services. A superbill helps you request reimbursement from your insurance company. Reimbursement depends on your specific plan and is not guaranteed.

What Is a Superbill?

A superbill is an itemized statement of the services your child received. It includes information used to process an insurance claim, such as the patient’s name, provider information, dates of service, procedure and diagnosis codes, and charges.

You submit the superbill to your insurance company to request reimbursement. Your insurer may also require a claim form, proof of payment, or additional documentation. A superbill alone may not complete your claim. See an example of an insurer’s claim requirements.

How to Request a Superbill from Rooted

Rooted provides superbills monthly, by request only. Superbills are not sent automatically. To request your monthly superbill, contact admin@rootedtherapies.com.

For this reimbursement process, families submit their superbills and follow up directly with their insurance company. Payment to Rooted remains due according to our payment policy, regardless of when insurance processes your claim.

Check your insurer’s filing deadline and request your superbill with enough time to submit each visit.

Before Starting Services: Check Your Benefits

Call the member services number on your insurance card and ask:

  • Does my plan cover out-of-network speech, feeding, or occupational therapy for my child?

  • Are evaluations and ongoing treatment both covered?

  • Is Rooted and my child’s specific treating provider eligible for reimbursement under my plan?

  • Is a referral or prior authorization required?

  • Are there diagnosis restrictions, visit limits, or combined therapy limits?

  • What is my out-of-network deductible, and how much remains?

  • How do my individual and family deductibles work?

  • After the deductible, what percentage does my plan pay, and what allowed amount is that percentage based on?

  • How do I submit a claim, what documents are required, and what is the filing deadline?

  • How long does processing usually take?

Keep the representative’s name, call date, and reference number. Request written benefit information when available. A benefits estimate does not guarantee payment; the insurer reviews the actual claim against your coverage requirements. Learn more about claim review and coverage requirements.

How Deductibles and Reimbursement Work

Many plans require you to meet an out-of-network deductible before they begin paying for covered out-of-network services. This deductible may be separate from your in-network deductible.

Your insurer may calculate reimbursement using its allowed amount, which can be lower than the amount you paid. Meeting your deductible does not necessarily mean your plan will reimburse the full session fee. Learn how deductibles, coinsurance, and coverage limits work.

If you or family members receive other out-of-network care—such as physical therapy, counseling, or chiropractic services—request itemized statements and submit eligible claims according to your plan’s instructions.

Not every cash-pay expense counts toward your deductible. Only amounts your plan recognizes as eligible may apply. Family members’ expenses may contribute to a family deductible when they are covered under the same plan, but individual deductible rules still matter. Ask your insurer how each claim will be applied. See examples of individual and family deductible rules.

How to Submit Your Superbill

  1. Review it. Check your child’s name, dates of service, and other details.

  2. Follow your insurer’s instructions. Complete any required claim form and attach the superbill and other requested documents.

  3. Submit before the deadline. Use your insurer’s approved portal or mailing process.

  4. Keep a copy. Save the superbill, submission confirmation, and claim number.

  5. Review the result. Your Explanation of Benefits, or EOB, shows how the claim was processed, including any payment or amount applied to your deductible. An EOB is not a new bill. Learn about claims and EOBs.

How Long Does Processing Take?

Plan for several weeks. Four to six weeks can be a reasonable planning estimate, but it is not a guaranteed timeline. Processing varies by insurer, submission method, and whether additional information is needed. Some insurers publish a four-to-six-week timeframe; confirm yours directly. See AmeriHealth’s claims-processing guidance.

Contact your insurer if its stated processing window has passed. Contact Rooted if the insurer identifies missing or incorrect information on your superbill.

Frequently Asked Questions

Does receiving a superbill mean insurance will reimburse me?

No. Your plan must cover the service and permit reimbursement for the provider. Deductibles, authorization requirements, coverage exclusions, and other plan rules affect payment.

Can I use a superbill with Cigna, Aetna, or Blue Cross Blue Shield?

Potentially. If Rooted is out of network with your specific plan, ask whether that plan accepts member-submitted claims for your child’s therapy. The insurance company’s name alone does not establish coverage. Some plans offer no routine out-of-network benefits.

Can I submit claims before meeting my deductible?

Yes, if your plan accepts claims for those services. An eligible claim may apply toward your deductible even when it produces no reimbursement. Submit within the filing deadline rather than waiting until you believe the deductible has been met.

Do all of my family’s cash-pay services count toward our deductible?

Potentially. The service, provider, covered family member, and amount must qualify under your plan’s rules. Amounts above the insurer’s allowed amount may not count. Read more about out-of-network charges.

Does Rooted submit the superbill for me?

For the superbill reimbursement process described here, you submit the claim directly to your insurance company. Rooted provides the superbill and can address questions about the information listed on it.

Can I use a superbill with TRICARE?

TRICARE has separate requirements. The treating provider must be TRICARE-authorized for TRICARE to pay any part of the claim, and referral or authorization rules depend on your plan. Contact your regional contractor and Rooted before starting services; a superbill alone does not establish eligibility. Review TRICARE’s provider requirements.

What if my child has Medicare or Medicaid?

Contact Rooted before scheduling private-pay services, including when Medicaid is secondary insurance. These programs have specific provider, billing, and patient-payment rules.

A general waiver does not automatically make private billing permissible or create a right to reimbursement. Medicare does not allow speech-language pathologists or occupational therapists to opt out through its private-contracting process. Texas Medicaid also places conditions on when a patient may be charged. Any permitted private-pay arrangement and required documentation must be reviewed before services begin. What should I do if my claim is denied?

Review the reason on your EOB and contact your insurer. Ask whether the claim needs corrected information, additional documentation, or an appeal, and confirm any deadlines. If the issue involves information on Rooted’s superbill, contact admin@rootedtherapies.com.

Will I automatically receive a superbill each month?

No. Rooted provides superbills monthly, by request only. Email admin@rootedtherapies.com to request your monthly superbill, allowing enough time to meet your insurer’s filing deadline.
